Occupational Therapist (OT) - Sioux Falls, SD
Blue Stone Therapy is seeking an Occupational Therapist (OT) to provide rehabilitation services to adult and geriatric patients at Dow Rummel in Sioux Falls, SD.
This role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Occupational Therapist is responsible for assessments, care plan development, and implementation of treatment programs designed to improve activities of daily living (ADLs), cognitive function, upper extremity strength, range of motion, positioning, and overall functional independence.
This position will serve multiple partner communities with travel between locations as needed.
What You'll Do
Perform occupational therapy evaluations and develop individualized plans of care
Provide skilled occupational therapy treatment focused on functional and measurable outcomes
Address ADLs, functional mobility, cognition, strength, positioning, and discharge planning
Supervise Occupational Therapy Assistants in accordance with regulatory and company standards
Maintain appropriate patient schedules per established guidelines
Complete timely, accurate documentation to support quality care and reimbursement requirements
Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes
Requirements
What You Bring Graduate of an accredited Occupational Therapy program
Current Occupational Therapist license (or eligibility for licensure) in applicable state
Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred
Strong clinical reasoning, communication, and organizational skills
Commitment to patient-centered care and professional standards
