Weare seeking a skilled and compassionate Occupational Therapisttojoin a Skilled Nursing Facility inAtascadero,California. This role focuses on helpingpatients regain independence in daily living activitiesfollowingillness, injury, or surgery. You will work with ageriatric andpost-acute population, developing personalizedtreatment plans thatimprove functional ability and overallquality of life. JobResponsibilities:
•Evaluate and treat patients withphysical, cognitive, and functional limitations in a SkilledNursingFacility setting
•Develop and implementindividualizedoccupational therapy treatment plans
•Train patients inactivities of daily living (ADLs) such asdressing, bathing, andfeeding
•Recommend adaptiveequipment and environmentalmodifications to improveindependence
•Monitor patient progressand adjusttreatment plans as needed
•Maintain accurate andcompliant documentation of patient care and outcomes
Requirements
•Active state license as an Occupational Therapist inCalifornia
•Current BLS/CPR certification
•Minimum of1year of recent OT experience preferred, ideally in a SNF orgeriatric setting
•Strong problem-solving and patient careskills
•Ability to work collaboratively within amultidisciplinary team
