Weare seeking a skilled and compassionate Occupational Therapist tojoin a Skilled Nursing Facility in San Jose,California. This role focuses on helpingpatients regain independence in daily living activities followingillness, injury, or surgery. You will work with a geriatric andpost-acute population, developing personalized treatment plans thatimprove functional ability and overall quality of life. Job Responsibilities:
•Evaluate and treat patients withphysical, cognitive, and functional limitations in a Skilled NursingFacility setting
•Develop and implement individualizedoccupational therapy treatment plans
•Train patients inactivities of daily living (ADLs) such as dressing, bathing, andfeeding
•Recommend adaptive equipment and environmentalmodifications to improve independence
•Monitor patient progressand adjust treatment plans as needed
•Maintain accurate andcompliant documentation of patient care and outcomes
Requirements
•Active state license as an Occupational Therapist inCalifornia
•Current BLS/CPR certification
•Minimum of 1year of recent OT experience preferred, ideally in a SNF orgeriatric setting
•Strong problem-solving and patient careskills
•Ability to work collaboratively within amultidisciplinary team
