Job description
Enhance Rehabilitation is seeking an Occupational Therapist to join our dynamic team in a SNF setting. You will evaluate and treat patients following individualized plans of care, and set measurable functional goals.
You will collaborate with facility staff to ensure excellent patient care and provide education on injury, progress and outcomes. You will re-evaluate progress, revise plans of care, and ensure timely documentation of evaluations, re-evaluations, discharges, and daily progress
