Position Overview
nThe Occupational Therapist is responsible for evaluating patients in their homes and developing individualized therapy plans that help restore daily living skills, independence, safety, and functional mobility .
nYou will work closely with patients recovering from illness, injury, surgery, or chronic conditions to improve their ability to perform everyday activities.
Key Functions & Responsibilities
nPatient Evaluation & Assessment
nnnConduct comprehensive in-home occupational therapy evaluations
n
nnAssess patient ability to perform Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs), Assist patient with hygiene while educating and training caregivers and loved ones on assisting with bathing and self-care.
n
nnReview physician orders, medical histories, and diagnoses
n
nnIdentify functional deficits and establish patient-centered goals
n
n nTreatment Planning & Implementation
nnnDevelop individualized treatment plans tailored to each patient's functional needs
n
nnImplement therapeutic interventions to improve strength, coordination, balance, and independence
n
nnTrain patients in adaptive techniques to improve daily living skills
n
nnRecommend and train patients on the use of adaptive equipment or assistive devices
n
n nPatient & Caregiver Education
nnnEducate patients and caregivers on safe mobility, home safety, and fall prevention
n
nnProvide instruction on energy conservation techniques and body mechanics
n
nnDevelop personalized home exercise and functional training programs
n
n nProgress Monitoring
nnnMonitor and document patient progress toward therapy goals
n
nnAdjust treatment plans based on patient response and outcomes
n
nnCommunicate regularly with physicians and the interdisciplinary care team
n
n nCollaboration
nnnWork closely with nurses, physical therapists, physicians, and case managers
n
nnParticipate in care coordination to ensure comprehensive patient care
n
nnProvide recommendations for continued therapy or discharge planning
n
n nDocumentation & Compliance
nnnMaintain accurate and timely patient documentation
n
nnComplete all required records for billing, compliance, and insurance
n
nnEnsure compliance with HIPAA regulations and agency policies
n
n Service Areas
nWe are currently seeking therapists to serve patients throughout Maricopa County , including:
n• West Valley
• Paradise Valley
• North Scottsdale
• Central Phoenix
• Peoria
• Glendale
Qualifications
n✔ Active Arizona Occupational Therapist (OT) License
✔ Home health experience preferred but not required
✔ Strong clinical assessment and problem-solving skills
✔ Excellent communication and patient education abilities
✔ Ability to work independently and manage a field caseload
✔ Passion for helping patients regain independence and quality of life
