Job Title: Social Worker

Description:
Provide social work services including psychosocial assessment, support, and discharge planning.
Conduct interviews to gather relevant client information.
Provide counseling to patients and families.
Provide information and referrals to appropriate community services.
The position will involve working with both adults and pediatric populations.
