nnnDeliver personalized, evidence-based therapy: Implement tailored treatment plans using appropriate modalities to help patients regain independence and improve their quality of life.
n
nnTrack and communicate progress: Document patient journeys and collaborate closely with the Occupational Therapist to revise goals and adapt treatment plans based on real-time responses.
n
nnEmpower patients and families: Educate individuals and their caregivers on managing deficits, building confidence for a safer transition home.
n
nnGuide seamless transitions: Coordinate discharge planning, recommending adaptive equipment and community resources to ensure ongoing success beyond clinical care.
n
nnMentor support staff: Oversee therapy technicians to maintain high safety standards, seamless workflow, and quality care delivery across the team.

n nWhat minimum requirements you'll need
nLicensure / Certification / Registration:
nnnBLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
n
nnCertified Occupational Therapy Assistant (COTA) credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date required.
n
n nEducation:
nnHigh School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
