n**8-hour Day Shift - Mileage Reimbursement (58.5 cents per mile)- No After Hour On Call Requirements

We are seeking a skilled and compassionate Occupational Therapist Assistant to join the Deaconess Home Health team. The OTA will work collaboratively with the OT and other interdisciplinary team members to plan, implement and evaluate occupational therapy services for patients in their homes. ******* *NestMed ambient AI technology ********
nResponsibilities:
nnnAssess patients' physical, cognitive, and psychosocial abilities to determine therapy needs.Develop and implement personalized treatment plans that may include exercises', adaptive techniques, and use of assistive devices to improve patients functional abilities.

nnTeach patients and family members about home safety, adaptive equipment, and strategies to manage daily activities effectively.
n
nnMaintain accurate and timely records of patient progress, treatment plans, and modifications to therapy programs for compliance and regulatory standards.
n
nnMonitor patients progress and adjust treatment plans as necessary to meet changing needs. Utilize outcome-based measures to assess the effectiveness of therapy interventions.
n
nnCollaborate and communicate patient's change in condition with physicians, nurses, physical therapists, speech therapists, and other home health team members.
n
n nJob Requirements
nnnAssociates of applied science (AAS) degree in occupational therapy assistant studies or a related field.
n
nnActive registered OTA license in the state which you practice
n
nnMust obtain and maintain current CPR/BLS Certification
n
nnOne-year Occupational Therapy Assistant experience in a clinical setting.
n
nnCurrent Drivers License
n
nnComply with federal, state and local laws and regulations

n$27.43 - $40.80 /hour
