Job Description
Pharmacist - Clinical
Schedule
Varied shifts, typical shift from 12:30 PM to 9 PM. Must be willing to cover weekends and evening shifts.
Assignment Details
Full-time position, 40 hours per week with variable weekend hours depending on staffing needs.
Requirements
Active Pharmacist license in good standing in the state
Minimum of 1 year of inpatient hospital pharmacy experience
Experience with pharmacokinetic dosing and warfarin management
Experience with Epic Willow Inpatient preferred but not required
Responsibilities Provide medication management and review for inpatients
Collaborate with healthcare teams to ensure optimal patient outcomes
Monitor patient responses to medication therapies
Maintain accurate patient medication profiles
