Job description
Gentiva is seeking an Afterhours RN Care Manager to provide high-quality hospice care after hours, including nights, weekends, and on-call shifts. You will conduct assessments, admissions, and routine visits, maintaining primary clinical oversight and collaborating with physicians and the IDG.
You will educate patients and families on disease progression, symptom management, and hospice philosophy while ensuring accurate documentation and compliance across the care continuum.
