Job description
Hartford HealthCare at Home is seeking an On-Call Visiting Nurse to provide skilled nursing care in patients' homes in the Hartford area. The role requires adapting to an on-call schedule from 4pm to 8am, including evenings, nights, weekends, and holidays.
You will assess patients, plan and implement care, coordinate with physicians and case managers, and lead LPNs and Home Care Aides to deliver high-quality, patient-centered care. Experience in home health is preferred.
