Job description
Eden Health Hospice in Longview, WA is seeking a dedicated Registered Nurse to serve as a Case Manager from admission through discharge, coordinating care with the interdisciplinary team to ensure compassionate, high‑quality end‑of‑life support. You will assess patient needs, educate families, manage the plan of care, and advocate for comfort and dignity while handling caseloads of 12–18 patients.
