Job description
Grace Healthcare Services is actively hiring for an On-Call Hospice Visit Nurse (RN/LPN) to provide compassionate, on-site hospice care across Middlesex, Union, and Somerset Counties. This full-time role features evening-to-night shifts on a 7-on/7-off schedule, working with the RN Case Manager and Interdisciplinary Team to deliver patient- and family-centered care.
Candidates must hold an active RN or LPN license and have hospice, palliative, or acute care experience.
