Job description
Crossbridge Hospice, LLC in Cincinnati, OH, is seeking an On-call RN Case Manager to deliver end-of-life hospice services in patients' homes or community settings. You will provide intermittent skilled nursing, supervise hospice aides, and coordinate care with the triage team.
