Job description
Bluestone Physician Services is seeking a Nurse Practitioner or Physician Assistant to deliver on-site primary care in Assisted Living and Memory Care communities around Minneapolis, MN. You will work within a multidisciplinary team to design and manage individualized care plans, supporting patients with chronic conditions and coordinating with LPNs, RNs, and social workers.
