Job description
CenterWell Home Health in Grand Junction, CO is seeking a Registered Nurse Clinical Manager to lead a team of clinicians delivering high-quality in-home care. You will guide clinical practice, ensure Medicare/CMS compliance, and drive quality improvement across the branch.
This role requires an active RN license, CPR, a valid driver's license, and at least 2 years of traditional home health experience; OASIS and HCHB proficiency plus PDGM knowledge are highly preferred.
