Job description
ALC Home Health is seeking a PRN Physical Therapist to deliver home health PT on the road under a physician-approved plan of care. The role includes patient assessment, treatment planning, documentation, and coordination with the Home Health Director.
The position offers flexible scheduling with control over geographic area, plus administrative support and EMR-enabled visit tracking. State licensure and two years of PT experience are preferred.
