Job description
Athena Home Health & Hospice seeks a compassionate Hospice RN Case Manager to plan, organize, and direct hospice care services for patients at home and in communities in Southern New England.
The role covers Litchfield County and western Hartford County. It is a salaried position with weekly pay up to $49 per hour, with a Monday–Friday 8:00 a.m. to 4:30 p.m. schedule. Relocation is the reason for vacancy.
