Job description
University of California - Los Angeles Health seeks a patient service liaison to support physicians and patients in assigned care teams. You will act as a liaison for patients and physicians, help patients access services within the health system, and facilitate communication with referring physicians.
Role includes referring patients to appropriate resources and providing follow-up via telephone and EPIC/Care Connect to ensure timely care and coordination across the care continuum.
