Job description
The American Oncology Network, Inc. is seeking an onsite LVN/LPN Nurse Navigator in Houston to coordinate oncology care, educate patients, and advocate for timely access to services.
Under physician and RN leadership, you will identify navigation needs, coordinate referrals across hospitals, infusion, home health and hospice, document in the EMR, and support transitions of care. Strong communication and care coordination skills are essential.
