Job description
LiveWell Homecare Agency, LLC seeks a Nurse Navigator to coordinate patient care across the treatment continuum. The role collaborates with patients, families, physicians, and interdisciplinary teams to ensure effective treatment and support.
Key duties include patient education, community outreach, and support group facilitation, particularly in oncology settings. Requires RN license, BLS, and 2+ years of related experience. BSN preferred with oncology certifications a plus.
