Job description
DCH Health System seeks a Clinical Pharmacy Specialist to support specialty pharmacy patient care. The role acts as a liaison between specialty staff and Pharmacy leadership, delivering services across access, delivery, and clinical care for patients on specialty medications.
Responsibilities include patient assessment, medication review, education, and regular follow-up, with emphasis on pharmacokinetics, dosing, stewardship, and safety. Precepting and training occur as part of the job.
