Job description
Kaiser Permanente in Baldwin Park, CA is seeking a RN Case Manager to collaboratively oversee a panel of physicians and manage oncology/radiology patients. You will assess needs, develop care plans, and coordinate services across multiple providers to ensure quality outcomes.
You will monitor plans, facilitate transfers, and liaise with external agencies while educating patients and families to empower self-management and adherence to treatment plans.
