Job description
Infinite Medical P.C. is hiring a Primary Care Nurse Practitioner/Physician’s Assistant to provide on-site care to residents in long-term care facilities in Hornell, NY. This is a full-time position designed for providers who value meaningful patient relationships and collaborative care.
In this on-site role you will conduct exams, manage chronic and acute conditions, order tests, develop care plans, and work with facility staff and interdisciplinary teams to deliver elder-centered care.
