Job description
Central Health is seeking an experienced Registered Nurse Case Manager - Transitions of Care to lead care coordination for patients with complex conditions during critical transitions. This onsite role will require collaboration across inpatient, outpatient, and community settings to reduce readmissions and improve outcomes.
Ideal candidates will have ADN, RN licensure in Texas, and 3+ years of clinical nursing with case management experience.
