Job TitleProvide coordination and management of service delivery for people with serious mental illness and/or intellectual/development disabilities, possibly with co-occurring substance use disorders.Application Period: 10/13/ /20/2026Location: Based in Ontonagon with travel to other countiesSalary: $48,695 – $63,886 with competitive benefitsDuties: This position requires interactions with various disciplines, families, staff and supervisors as well as consumers. Requires collaboration with a multi-disciplinary team in assessment, diagnosis and person-centered planning to perform biopsychosocial assessments, develop individual plans of service, assess and monitor progress towards goals and objectives and maintain confidential records.Minimum Qualifications:Degree in Social Work, however, other Human Services degrees will be considered;1 year experience working with those with intellectual/developmental disabilities and/or mental illness;Ability to work in Ontonagon County, Michigan, and travel to surrounding counties as needed;Strong interpersonal skills;Ability to work as a team member;Able to pass a criminal background check;Valid Michigan driver's license and dependable vehicle.Desirable Qualifications:Michigan Licensure as a Bachelor's or Master's Social Worker.Qualifications of a Child Mental Health Professional.
