Job description
Salary: $60-80+/hr based on experience
nLaSante Health Center is seeking an Optometrist to provide comprehensive eye care services in a federally qualified health center (FQHC) setting on per diem basis. This per diem role focuses on delivering high-quality vision care while collaborating with primary care and specialty teams to support overall patient health.
nResponsibilities:
nnnPerform comprehensive eye examinations and vision assessments

nnDiagnose and manage common eye conditions and vision disorders
n
nnPrescribe corrective lenses and contact lenses as appropriate
n
nnIdentify and refer patients with complex or surgical needs to specialists
n
nnEducate patients on eye health, preventive care, and treatment plans
n
nnDocument all encounters accurately in the electronic health record (EHR)
n
n nQualifications:
nnnDoctor of Optometry (OD) degree from an accredited program
n
nnCurrent New York State optometry license (or eligibility)
n
nnExperience in outpatient or community health settings preferred
n
nnStrong communication and patient engagement skills
n
n
