Job description
Join Cleveland Clinic Fairview Hospital as an RN Care Coordinator in Orthopedics Trauma, providing care coordination for a high‑risk surgical population. You will collaborate with multidisciplinary teams to optimize outcomes and reduce readmissions.
Must have RN licensure, BLS, and 3–5 years of nursing experience; BSN preferred. Work mainly 7:30am–5:00pm, with occasional float to other facilities within the system.
