Job description
Mercy in Missouri is seeking a Nurse Navigator to lead and coordinate patient care across inpatient and outpatient settings. You will assess care needs, guide care plans, educate patients, and ensure timely, safe, and high-quality delivery of services.
The role emphasizes collaboration with bedside nurses, adherence to standards, discharge planning, and performance metrics to optimize LOS and utilization. RN licensure and 2–3 years of experience are preferred.
