Job description
Hartford Hospital is seeking an Orthopaedic Trauma Nurse Navigator to support patients through the continuum of care. You will advocate, educate, and coordinate in collaboration with surgeons and multidisciplinary teams to ensure seamless transitions from acute care to outpatient services.
The role requires a BSN and CT RN licensure, with two years of clinical nursing experience; a Master’s degree is preferred.
