Job description
Hartford Hospital is seeking an Orthopaedic Trauma Nurse Navigator to support patients through the continuum of care. This role collaborates with a multidisciplinary team as a patient advocate, liaison, and educator to ensure seamless transitions and high-quality outcomes.
Qualified candidates hold a BSN with two years of clinical nursing experience and an active CT RN license; a Master’s degree is preferred.
