Job description
Hartford HealthCare is seeking an Orthopaedic Trauma Nurse Navigator at Hartford Hospital to support patients across the care continuum. You will advocate, liaise and educate in collaboration with multidisciplinary teams.
Requirements include a BSN, active CT RN license, and 2 years of clinical nursing experience; Master's degree is preferred. This role participates in quality improvement projects and engages patients and families for optimal outcomes.
