Job description
Occupational Therapist (OT)
Evaluate residents’ functional abilities, including activities of daily living (ADLs), mobility, cognition, and psychosocial status.
Develop and implement individualized, evidence-based treatment plans in accordance with evaluation findings and physician orders.
Deliver direct occupational therapy services in compliance with state and federal regulations as well as facility policies.
Monitor, assess, and document resident progress, updating treatment plans as needed to achieve rehabilitation goals.
Collaborate and participate in interdisciplinary care planning meetings to ensure optimal resident outcomes.
Educate residents, families, and caregivers on techniques to enhance safety and functional independence.
Recommend and train on adaptive equipment and assistive devices to support resident needs.
Supervise and provide guidance to Certified Occupational Therapy Assistants (COTAs) and therapy aides as appropriate.
Maintain accurate, timely, and compliant documentation in the electronic medical record system.
Stay current with licensure and continuing education requirements as mandated by the state.
Support quality improvement initiatives and ensure adherence to CMS and facility guidelines.
Work within a skilled nursing/long-term care environment, often requiring physical tasks such as standing, lifting, and transferring residents in a collaborative, fast-paced setting.
