Job description
Job Summary (List Format):
- Evaluate residents' functional abilities, including activities of daily living (ADLs), mobility, cognition, and psychosocial factors.
- Develop and implement individualized treatment plans based on evaluations and physician orders.
- Provide direct, evidence-based occupational therapy services in compliance with state, federal, and facility regulations.
- Monitor, document, and adjust resident progress and treatment plans to achieve rehabilitation goals.
- Collaborate with an interdisciplinary team, residents, and families to ensure optimal rehabilitation outcomes.
- Educate residents, families, and caregivers on strategies to enhance safety and independence.
- Recommend and instruct in the use of adaptive equipment and assistive devices as needed.
- Supervise Certified Occupational Therapy Assistants (COTAs) and therapy aides as appropriate.
- Ensure timely, accurate, and compliant documentation in electronic medical records.
- Maintain licensure and complete continuing education as required by state regulations.
- Support quality improvement initiatives and adhere to CMS and facility guidelines.
- Work in a skilled nursing facility/long-term care setting, which may require physical activity such as standing, lifting, and transferring patients.
- Function effectively within a collaborative, fast-paced interdisciplinary environment.
