This is a 13-week contract position.
Job Description
The Home Health Registered Nurse will provide skilled home care visits to patients with diverse medical needs, including post-surgical recovery and chronic conditions. The role involves direct patient care and coordination with interdisciplinary teams to ensure optimal health outcomes.
Responsibilities
Conduct comprehensive patient assessments and develop care plans.
Provide nursing care for patients with conditions such as cardiac, orthopedic, respiratory, and neurological issues.
Administer medications and treatments as prescribed.
Perform OASIS assessments and document care using BayCare Forms and scheduling tools.
Manage up to 8 patient visits per day, ensuring high standards of care.
Educate patients and families on health management and discharge planning.
Maintain compliance with health regulations and safety protocols.
Required Experience / Certifications / Licensure Minimum of 1 year of home health experience.
Valid RN license in Florida or Compact state.
BLS certification required.
OASIS training preferred.
Must possess a valid driver's license and have access to a car for home visits.
COVID-19 vaccination required or approved exemption on file.
Flu shot required or declination with mandatory mask wearing while at work.
