Board Eligible or Board Certified in Otolaryngology - REQUIRED
Current or Willing to obtain Colorado Medical License - REQUIRED
Interest in practicing General ENT full-time - REQUIRED
Availability/start date - REQUIRED
Clean malpractice/license history - HIGHLY PREFERRED
License history and disciplinary history - REQUIRED
Candidate's subspecialty interests (Laryngology, Skull Base Surgery, etc.) - HIGHLY PREFERRED
Clean malpractice/license history - HIGHLY PREFERRED
Most recent NPDB Self Query Report (within 30 days active) - REQUIRED
DEA (must be obtained prior to start) - REQUIRED

Annual compensation range: $458,000 - $508,000
