Job description
Holy Cross Hospital in Fort Lauderdale, FL seeks a Diabetes Care & Education Specialist to deliver diabetes self-management education and support to patients in outpatient settings. The role collaborates with clinicians to design individualized education plans and improve patient outcomes.
The Specialist will assess patient needs, provide education in one-on-one, classroom, and telephonic formats, and work toward CDE certification within the first year.
