Job description
Valor Healthcare, Inc. is seeking a Primary Care Nurse Practitioner to provide prescribed medical treatment and personal care services to patients in the clinic, under the direction of physicians or mid-level providers.
You will collaborate with the core PACT Team (Primary Care Provider, RN and Medical Assistant) and expanded PACT Team including VA and community-based services to develop the patient care plan.
