Job description
Montare Behavioral Health in Los Angeles is seeking a Case Manager for our Mental Health and Substance Use Outpatient Treatment Program. You will provide direct support, care coordination, treatment engagement, discharge planning, and linkage to community resources.
You will work with a multidisciplinary team to help clients build stability, increase independence, and transition to lower levels of care, while documenting in the EMR and promoting a safe, therapeutic environment.
