Our client in Oregon is seeking an experienced and dedicated on-site Outpatient Pharmacist to join their team.

Requirements of the Outpatient Pharmacist:
Outpatient pharmacy background and experience
Must be a registered Pharmacist, BS, or PharmD
Direct-hire Opportunity
BLS Basic Life Support
Responsibilities of the Pharmacist: Prepare and dispense medications using appropriate techniques and following the policies and procedures.
Design, recommend, monitor, and evaluate patient-specific pharmacotherapy.
Provide concise, applicable, and timely responses to requests for drug information from healthcare providers and patients.
Provide medication-use education to patients and caregivers.
Utilize pharmacy support personnel effectively.
Ensure continuity of pharmaceutical care to and from the acute and ambulatory patient-care settings.
Participate in the established process for assessing, managing, and reporting medication errors.
Participate in the management of medical emergencies.
