nPhysical Therapist (Per Diem) – Putnam Hospital Center – Yorktown/Jefferson Valley, NY
nSchedule: As needed, Monday–Friday, 8-hour shifts between 7:00 AM – 8:00 PM
nLocation: Yorktown–Jefferson Valley Professional Building, 2585 Navajo Street
nOverview:
nPutnam Hospital Center is seeking a Per Diem Physical Therapist (PT) to join our Physical Therapy Satellite Department . In this role, you will evaluate, treat, and educate patients of all ages—pediatric through geriatric—following a medically prescribed physical therapy plan. You’ll work with individuals recovering from injury, illness, or surgery to restore movement, reduce pain, and help them achieve optimal function and independence.
nResponsibilities:
nnn nPerform comprehensive physical therapy evaluations and develop individualized treatment plans.
nnn nProvide hands-on patient care focused on recovery, mobility, and long-term wellness.
nnn nDocument patient progress and maintain accurate records in the electronic medical record (EMR) system.
nnn nEducate patients and families on exercises, safety, and rehabilitation goals.
nnn nCollaborate with physicians and interdisciplinary teams to ensure continuity of care.
nQualifications:
nnn nRequired: Bachelor’s Degree in Physical Therapy from an accredited program.
nnn nPreferred: Master’s or Doctor of Physical Therapy (DPT) degree.
nnn nPreferred Experience:
nnn n1+ year of professional physical therapy experience.
nnn nExperience with EMR systems.
nnn nBackground in orthopedics, neurology, or vestibular rehabilitation.
nnn nSkills: Excellent communication, critical thinking, and patient-centered care abilities.
nnn nLicensure: Active New York State Physical Therapist License or eligibility for a temporary permit.
nWorking Conditions:
nnn nManual dexterity and fine motor coordination required.
nnn nModerate to significant physical effort; potential exposure to typical clinical environment elements (e.g., odors, noise).
nCompensation:
nnn n$67.00 per hour flat rate
