Job description
Hartford HealthCare is seeking a Full-time Family Nurse Practitioner or Physician Assistant for a multi-provider, primary care practice serving patients age 12 and up. The role is based in Plainfield and Putnam, CT, with outpatient primary care duties.
Open to FNPs and PAs, the position offers a four-day work week across Plainfield and Putnam, with no on-call requirements. Licensure and APN/PA training required, plus two years of post-graduate experience in primary care.
