Community Care Physicians seeks an Outpatient Nurse Practitioner or Physician Assistant to join our physician-owned multispecialty group in Latham, NY. In this role, you will provide patient-centered primary and specialty outpatient care, including comprehensive assessments, chronic disease management, and acute visits.
