Job description
Community Care Physicians seeks an Outpatient Nurse Practitioner or Physician Assistant to deliver high-quality, patient-centered care in our multispecialty medical group. In this role, you will manage acute and chronic conditions, perform comprehensive assessments, order and interpret diagnostics, and prescribe medications within a collaborative, team-based environment. You will use an EHR, participate in care coordination and quality improvement, and help advance our mission of accessible, community-focused care across New York’s Capital Region.
