Job description
Community Health Connections, Inc. is seeking a Primary Care Nurse to provide comprehensive outpatient nursing care, focusing on health promotion, disease prevention, and patient-centered management across the lifespan.
You will take vitals, assist during exams, administer medications and immunizations, coordinate care, support transitions, educate patients, and document in the EHR while upholding CHC values. Some evenings may be required.
