Job description
Dignity Health is seeking a Transitional Care Nurse Practitioner or Physician Assistant for an outpatient clinic in the Phoenix area. You will provide comprehensive primary care, including diagnosis, management of common illnesses, and coordination with a multidisciplinary team.
Licensed NP or PA with adult internal medicine expertise, strong clinical skills, and ability to collaborate across care teams is required.
