Job description
Community Care Physicians is seeking an Outpatient Vascular Nurse Practitioner or Physician Assistant to provide patient-centered specialty care in New York’s Capital Region. Evaluate patients with vascular concerns, obtain histories, perform examinations, develop treatment plans, order and interpret diagnostic studies, prescribe medications, and coordinate follow-up with physicians and care teams. Educate patients on conditions and treatment options, maintain accurate clinical records, and collaborate with colleagues to support quality improvement and continuity of care. Current New York licensure and national certification for the applicable role are required; vascular or surgical experience is preferred.
