Job description
Community Care Physicians is seeking an Outpatient Vascular Nurse Practitioner or Physician Assistant to provide patient-centered specialty care in New York’s Capital Region. In this role, you will assess patients with vascular conditions, review diagnostic findings, develop treatment plans, manage medications, educate patients, and coordinate follow-up care. Work collaboratively with physicians and a supportive clinical team across a physician-owned multispecialty group committed to quality improvement and strong patient relationships.
