Job description
The North East Medical Services in San Francisco, CA seeks a PACE Nurse Manager to lead nursing services across all PACE sites. You will supervise RNs, LVNs, Medical Assistants, and QI Coordinators, shaping staffing, training, and performance standards to deliver high-quality care.
Responsibilities include collaborating with PCPs, monitoring care metrics, guiding QA initiatives and audits, and coordinating home health, SNF, RCFE, hospice, and transitions of care.
