Job description
The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse is based at the Portsmouth Clinic in New Boston, OH, within the Primary Care Department. The RN delivers direct Veteran care and care coordination, using the nursing process to improve outcomes and access for patient panels.
Responsibilities include health promotion, disease prevention, and chronic care management, with leadership in interprofessional collaboration and evidence-based practice to enhance Veteran experiences.
